Certified Copies of Vital Records 

Customers can order a vital record by mailing the form (link below in green) or by coming in person to the Health Department.  Call for hours.

To get a copy of a vital record, you must submit:

  • A completed application  (form is link is in green, above)
  • A copy of the proof(s) of your identity
  • If you request records by mail and your ID does not have current address, you must send copy of secondary ID (see form).
  • The correct fee -  $25 for One certified copy, $2.00 for each additional certified copy of the same record, requested at the same time.  Payment in Cash ,Check or money order only
  • Proof of your relationship to the person listed on the vital record (If you are the mother or father on record, valid ID is sufficient)
  • Marriage certificate if your name has changed due to marriage

Registration of Vital Statistics 

The Health Department processes and maintains custody of the following documents related to Vital Statistics for Westwood Borough's population:

  • Registration of all Births, Deaths, Marriages, Civil Unions and Domestic Partnerships that occur in Westwood.
  • Acceptance of Marriage, Civil Union and Domestic Partnerships license applications.
  • Issuance of Burial Permits.
  • Filing of all Burial Permits from Westwood Cemetery.
  • Issuance of Certified Copies for Vital Records for any Birth, Death, Marriage, Civil Union or Domestic Partnership that has occurred in Westwood; following guidelines set forth in Executive Order Number 18.
