Community Health Needs Assessment


The Community Health Improvement Partnership (CHIP) of Bergen County, is a collaboration between public health departments, hospitals, community organizations and corporations.  It was established to develop, implement and evaluate strategies to improve the health in local municipalities.  One of the strategies that will be used to achieve this goal is the Community Health Needs Assessment.  Once community needs are identified, CHIP members will work together to create community programs and initiatives.   The CHIP is asking community members for their valuable assistance by completing the Survey. Your response is needed by August 18, 2022.  The Survey can be found by clicking here.
